Office Policies and Guidelines
Appointments
We respect the value of your time. We schedule and reserve appointment times specifically for you. Barring unexpected emergencies, we strive to see you promptly at your reserved time and do our best to uphold our reputation for “running on time.” In return, we ask that you arrive promptly and give us at least two days notice if you need to reschedule. We can then offer your appointment time to another patient needing help.
Appointments cancelled with less than 24 hours’ notice are subject to a $45 cancellation fee. Repeated missed appointments may require prepayment to reserve future appointments.
Dental Insurance
We are an out-of-network provider with all insurance carriers; however, we will gladly file claims on your behalf. Treatment estimates are provided as a courtesy and are not a guarantee of payment. Estimated patient portions (including deductible, co-insurance, and non-covered services) are due at the time of service. Failure to collect an estimated portion does not waive patient responsibility. Patients are financially responsible for all charges incurred, regardless of insurance coverage.
If insurance does not remit payment within 45 days, the outstanding balance becomes the patient’s responsibility. Any remaining balance after insurance payment is received is due within 15 days.
Card on File Authorization
To streamline billing and reduce administrative costs, Signature Dentistry of Macon provides the option to have a credit or debit card on file. After insurance processes, any remaining patient responsibility up to $100 may be charged to the card on file without additional notice. Balances exceeding $100 will require patient approval before processing.
If a card is declined, the patient agrees to provide updated payment information promptly. Maintaining a valid card on file may be required for scheduling future non-emergency appointments.
Payment Options
Payment is due at the time services are rendered unless prior written arrangements have been made. We accept cash, check, and major credit/debit cards. In-office financing is available for up to 90 days with drafted payment plans. For treatment costs requiring longer financing, Cherry and CareCredit are available.
Paid-in-Full Discount
A 5% paid-in-full discount is available for operative treatment when paid by cash or check
at the time of service.
Collections & Legal Action
Accounts with balances outstanding for more than 90 days may be referred to a third-party collection agency or pursued through small claims court. If referred, the patient agrees to pay reasonable collection costs, court costs, and attorney fees as permitted under Georgia law, including but not limited to a collection fee of up to 25% of the outstanding balance.
Accounts with balances over 60 days past due may accrue interest at the maximum rate permitted under Georgia law.
Any disputes regarding insurance processing or account balance must be submitted within 30 days of the statement date.
Signature Dentistry of Macon reserves the right to dismiss any patient for failure to comply with financial policies, broken payment agreements, or inappropriate conduct, with appropriate written notice and emergency coverage as required by Georgia law.
Minor Children
The adult accompanying a minor is responsible for payment of services. Minors must
always be accompanied by an adult.
